Grant Report

United Way is committed to investing in local programs producing measurable results.

This report is to be completed one year from the date dollars are granted, as indicated in your grant agreement.  If dollars are not completely disbursed after one year, a report must be submitted every six months until dollars are expended.  This form may be filled out for both Power of the Purse and Community Impact grants.
